Norwegian labor inspection authorities are criticizing Leroy Norway Seafoods for failures leading up to the death of an employee crushed in an ice machine at the company's whitefish processing plant in Stamsund, Norway, on Feb. 6.

Hussein Afzail, originally from Afghanistan, died during a routine cleaning of the equipment when a colleague inadvertently turned it on.

The day before the accident, the ice bin, used for both chilling in the processing plant and for vessels that deliver to the facility, was scheduled to be emptied, but ice was stuck inside one of the outflow pipes.
